If you have dental insurance through TennCare (Medicaid) but do not have additional IDD coverage (such as ECF, Waiver, or ICF), please note that our office only accepts patients with TennCare Medicaid who also have additional IDD or Medicare dental coverage.

For alternative dental care options, please contact Renaissance, the TennCare Dental Benefits Manager, to obtain a referral to another dental office.

Once we receive your completed form, we will review your chart and contact you with the next steps. This process typically takes no more than a few days.
